It should be filled in by Hami (Mentor) and sent to the following e-mail addresses.

Your mentors must send your documents numbered 1 and 2 only to the email address listed below.

The subject line of the email your mentor sends should be as follows. Don't forget to notify your mentor. Documents not sent in this manner will not be accepted.

3-)END OF TERM EVALUATION REPORT

4-)DEPARTMENT OF MECHANICAL ENGINEERING (ENGLISH) REPORT BOOK

All of the following rules must be followed for the internship report, which is form number 4.

• Everyone must comply with the rules written under the heading REPORT WRITING RULES.
• You must answer the questions under the heading BUSINESS GENERAL INFORMATION.
• You must answer the questions under the heading EVALUATION QUESTIONS. 
  

Regardless of your internship code, you must complete all the requirements listed under the relevant heading in the internship report.

• For example, students doing an internship with code WAP 227 must also answer the questions found in code WED 128.
• For example, students doing an internship with code WAP 327 must also answer the questions found in codes WED 128, WAP 227, and WAP 228.
• For example, students doing an internship for the WEX 427 code must also answer the questions in the WED 128, WAP 227, WAP 228, WAP 327, and WEX 328 codes.

• For example, students doing an internship for the WEX 428 code must also answer the questions found in the WED 128, WAP 227, WAP 228, WAP 327, and WEX 328 codes.

Each of your file names MUST be as follows:

3-) internship code + student number + name surname 

4-) internship code + student number + name surname

!! If you do not name your files correctly, your reports will not be accepted. !!
